Cleveland Clinic, University Hospitals, MetroHealth and Summa all run MBSAQIP programs, and every one of them will do a competent sleeve gastrectomy or gastric bypass. The differences are in everything around the operation: how long you wait, who you meet and when, which procedures are on the menu, whether you sleep in a hospital bed afterward, and what the bill looks like when there is no insurer in the middle. Cleveland Clinic Bariatric & Metabolic Institute

Cleveland Clinic is the biggest bariatric operation in Ohio by any measure. The institute lists eleven bariatric surgeons on its staff page, with four more at Akron General, operates at Main Campus, Fairview, Hillcrest, Lutheran and Akron General, reported 1,100 operations in its most recent year, and performs more than 95 percent of them with minimally invasive technique. It holds MBSAQIP accreditation along with Blue Distinction, Aetna, Cigna and Medical Mutual bariatric designations, and its director, Dr. Ali Aminian, published the 2025 Nature Medicine study showing that patients with obesity and diabetes who had surgery kept off 21.6 percent of their body weight at ten years against 6.8 percent for patients on GLP-1 medication alone. If you need a complex revision of a prior operation, or you are the parent of an adolescent who qualifies for surgery, this is the deepest bench in the region.

The trade-off is scale. The published pathway starts with an online seminar the institute asks every patient to complete, then a first surgical consult, then medical, psychological and nutritional clearances, all of which must be complete before surgery is scheduled, then a second pre-surgery visit and a two-week liquid diet. Cleveland Clinic assigns each patient a navigator as a single point of contact, which is a real advantage when it works. Patient reviews on ObesityHelp, where the institute carries a 1.5 out of 5 rating from fifteen reviewers, repeatedly describe the opposite: weeks waiting for callbacks, learning about insurance approval from the insurer months after the paperwork was done, and a four-week wait for a dietitian consult. The same reviewers praise the surgeons. The complaint is about the machine around them. Self-pay is offered but no price is published anywhere on the site, and no same-day discharge program is described.

University Hospitals

University Hospitals offers accredited surgical weight loss at UH Cleveland Medical Center, UH Geauga in Chardon, UH Lake West in Willoughby and UH Parma. The menu is broader than Cleveland Clinic’s public-facing list: gastric bypass, sleeve, duodenal switch, revision, robotic surgery, endoscopic sleeve gastroplasty and the six-month intragastric balloon. Dr. Leena Khaitan directs the UH Geauga program and serves as associate director at UH Cleveland Medical Center, and the system trains fellows in bariatric and foregut surgery. Dr. Karas practiced inside this system for years, built and re-accredited the UH Geauga program, and performed the first two Mag-DI cases ever done at University Hospitals in July 2026 before opening her own practice. She still holds privileges at UH Geauga and UH Geneva, so patients who need a hospital setting can have her operate there.

What UH does not offer is speed or same-day discharge. The first step is an online video session. When your insurer requires a supervised diet, UH satisfies it with monthly dietitian-led Zoom classes; its own page states that only one class per month counts, that each class must fall at least 21 days after the previous one, that the door locks ten minutes after start, and that some insurers restart the clock if a month is missed. Hospital stay is one to two nights. Self-pay is available with payment plans, and UH offers a bundled Center of Excellence+ package at a single rate to employers and payers, but no dollar figure appears on either page.

The six-month supervised diet is an insurance rule, not a surgical one

No requirement like it appears in the surgical literature, and no study shows that six months of mandatory visits makes a sleeve or bypass safer. Cash-pay patients at Karas skip the insurer clock and move from consult to surgery in six to ten weeks. Ask about the self-pay timeline.

MetroHealth Weight Loss Surgery and Weight Management Center

MetroHealth earned MBSAQIP Comprehensive Center accreditation for a program that has been running for more than fifteen years, with a surgical team led by Dr. Sergio Bardaro, who directs the program and has had gastric bypass himself, alongside Dr. Amelia Dorsey, Dr. Aneesah McClinton and Dr. Jeffrey Claridge. Robotic surgery has been in use since 2022. The medical weight management side is unusually strong for a public hospital, with seven obesity medicine providers prescribing GLP-1 therapy. Surgery happens at the main campus on MetroHealth Drive, and patients usually go home the next day.

The limitation is the menu. MetroHealth’s surgical page lists exactly two operations, sleeve gastrectomy and gastric bypass. There is no duodenal switch, SADI-S, balloon, endoscopic sleeve or magnet procedure. You need a referral from your primary care physician to start, the site does not publish a timeline from referral to surgery, and the billing page states that bariatric surgery is not eligible under the hospital’s financial assistance or discount program, though a no-interest pre-surgical payment plan exists. MetroHealth also cut about 125 mostly administrative positions in July 2025 after a $50 million operating loss in 2024, and while clinical services were described as unaffected, it is fair to ask any program how staffing changes reach the people who return your calls.

Summa Health, Southwest General and the rest of the region

Summa Health’s Weight Management Institute in Akron is MBSAQIP accredited, reports more than 4,000 weight loss operations over its history and 424 bariatric procedures in its most recent Leapfrog reporting year, and offers sleeve, bypass, SADI-S and revision. In January 2026 Dr. Adrian Dan performed the first MagDI case in Ohio there. Summa’s own page states an average of six to eight months from enrollment to surgery, with a required education seminar and a program administration fee that insurance does not cover. Private-pay patients must still carry major medical insurance.

Southwest General in Middleburg Heights hosts Dr. Leslie Pristas’s Best Life Bariatrics, a single-surgeon program offering sleeve, bypass, duodenal switch, SADI and band removal, with a stated wait of three to six months and a one to two day stay. Dr. Karas also holds privileges at Southwest General and uses it, along with Big Creek Surgery Center next door, for west side patients. Mercy Health runs smaller accredited programs in Lorain and at St. Joseph Warren Hospital, and Cleveland Clinic Akron General requires weekly support group attendance in addition to the online seminar and psychological counseling.

Karas Weight Loss & Wellness

Dr. Linden Karas is a board-certified general surgeon who completed her fellowship in bariatric and minimally invasive surgery at the Cleveland Clinic Foundation, then spent nine years building bariatric programs inside Northeast Ohio health systems. Building a program means writing the protocols, training the nursing staff, setting up dietitian workflows, collecting outcomes data and tracking every complication, and her work has been verified through MBSAQIP more than once. She opened Karas Weight Loss & Wellness on August 17, 2026, with offices in Lyndhurst and Westlake, an ownership interest in Legacy Surgery Center in Lyndhurst, and privileges at UH Geauga, UH Geneva and Southwest General for patients who belong in a hospital.

The practice was designed around the specific failures described above. Surgeon consultations are typically available within a week of referral, against the six to twelve weeks common inside the regional systems. Patients meet Dr. Karas a month or two into the process, before they commit, rather than at the last visit before surgery. She performs every operation herself. Body composition and metabolic testing happen every three months, sleep apnea and fatty liver are screened for up front, and the follow-up schedule she ran at UH, two weeks, six weeks, three months, six months, one year and then annually for life, carries over unchanged.

Same-day surgery in an accredited surgery center

Karas is the first same-day bariatric surgery practice north of Columbus. Qualified patients have a sleeve gastrectomy or gastric bypass at Legacy Surgery Center or Big Creek Surgery Center and go home the same evening. Same-day discharge after laparoscopic sleeve has been studied in national registry data for years. The largest MBSAQIP analysis found complication, reoperation and readmission rates no different from next-day discharge, and the rare deaths it did find clustered in patients with sleep apnea sent home without monitoring, which is exactly why patient selection is the whole game. Hospitals rarely offer it because an overnight stay produces more insurance revenue, which Dr. Karas has said plainly on this site. Patients with significant cardiac or pulmonary history, complex revisions, or a recovery that does not go as planned are kept overnight or scheduled at UH Geauga or Southwest General instead. The discharge decision follows the patient’s risk profile rather than the building’s revenue model.

What the bill looks like

Cleveland Clinic, University Hospitals and MetroHealth all offer self-pay bariatric surgery, and none of them prints a price. The reason is structural: a hospital bill stacks a facility fee on top of the surgeon’s fee, the anesthesia fee and the overnight room, and the total moves with every line. An ambulatory surgery center carries no hospital facility fee. Karas quotes one all-inclusive number in writing at the consultation, covering the surgeon, anesthesia, the facility and standard follow-up, and it comes in at roughly half of typical hospital pricing for the same operation. Insurance is accepted where the plan covers the service, and self-pay patients are not made to sit through an insurer-mandated supervised diet that no surgical society requires.
